基于国家免费药物的抗病毒疗法探索HIV感染后快速治疗对大脑结构和功能的影响Investigating the effects of rapid initiation on brain structure and function following HIV infection under the national free antiretroviral treatment program

摘要(Abstract):
目的 国家免费艾滋病抗病毒治疗(Antiretroviral Therapy, ART)药物方案仍然是当前中国推荐用于治疗HIV感染的主要方案。为探索HIV感染后ART对脑成像的影响开展此研究。方法 开展横断面研究,根据HIV感染诊断至启动ART的间隔时间来评估基于国家免费药物的快速启动ART(Rapid Initiation of ART,Rapid ART)对感染者脑结构和功能的影响。本研究招募了男男性行为人群HIV感染者,分为Rapid ART组(<1月)和非Rapid ART组(≥1月)两组。全部参与者接受了临床和多模态脑影像学的评估。结果 相比于非Rapid ART组,Rapid ART组的HIV感染者表现出脑总灰质及双侧楔前叶等特定脑区灰质体积更大,并且右侧距状裂周围皮层等脑区的低频振幅和局部一致性等脑功能指标发生改变。结论 本研究发现非Rapid ART组参与者相比于Rapid ART组参与者表现出明显的脑萎缩和功能改变。这或许将为接受国家免费抗病毒药物的HIV感染人群尽早启动ART提供潜在理论基础和数据支持。
